Employee Form
If you are a student and need to file a report about an employee (faculty or staff), or if you are an employee and need to file a report about another employee, please use the form below. You will need to print and complete the form and email it to hr@batestech.edu, or deliver it to Human Resources, Downtown Campus, Room 326, 1201 S. Yakima Ave., Tacoma, WA 98405. Questions?
